School Immunization Reminder
Ohio Requirements – Kindergarten, 7th & 12th Grade
Dear Parents and Guardians,
As we prepare for the upcoming school year, this is a reminder of the Ohio Department of Health immunization requirements for students entering Kindergarten, 7th grade, and 12th grade. These vaccines are required for school attendance.
Kindergarten Requirements
Kindergarten students must have documentation of the following vaccines prior to school entry:
DTaP (Diphtheria, Tetanus, Pertussis) – 5 doses
Polio – 4 doses
Hepatitis B – 3 doses
MMR (Measles, Mumps, Rubella) – 2 doses
Varicella (Chickenpox) – 2 doses or documented history of disease
7th Grade Requirements
Students entering 7th grade must have:
Tdap (Tetanus, Diphtheria, Pertussis) – 1 dose
Meningococcal (MCV4) – 1 dose
These vaccines are required prior to the start of school.
12th Grade Requirements
Students entering 12th grade must have:
Meningococcal (MCV4) – 2 doses total
OR 1 dose if given on or after the 16th birthday
Exemptions
Ohio law allows for medical or non-medical (religious/personal belief) exemptions from immunization requirements. If your child is not receiving the required vaccines, a signed exemption form must be completed and kept on file with the school each year.
