🏫 School Immunization Reminder

Ohio Requirements – Kindergarten, 7th & 12th Grade

Dear Parents and Guardians,

As we prepare for the upcoming school year, this is a reminder of the Ohio Department of Health immunization requirements for students entering Kindergarten, 7th grade, and 12th grade. These vaccines are required for school attendance.

📚 Kindergarten Requirements

Kindergarten students must have documentation of the following vaccines prior to school entry:

  • DTaP (Diphtheria, Tetanus, Pertussis) – 5 doses

  • Polio – 4 doses

  • Hepatitis B – 3 doses

  • MMR (Measles, Mumps, Rubella) – 2 doses

  • Varicella (Chickenpox) – 2 doses or documented history of disease

📘 7th Grade Requirements

Students entering 7th grade must have:

  • Tdap (Tetanus, Diphtheria, Pertussis) – 1 dose

  • Meningococcal (MCV4) – 1 dose

These vaccines are required prior to the start of school.

🎓 12th Grade Requirements

Students entering 12th grade must have:

  • Meningococcal (MCV4) – 2 doses total

    • OR 1 dose if given on or after the 16th birthday

⚠️ Exemptions

Ohio law allows for medical or non-medical (religious/personal belief) exemptions from immunization requirements. If your child is not receiving the required vaccines, a signed exemption form must be completed and kept on file with the school each year.
