We are excited to welcome our students back to school!
Open House will be held on August 20, 2026.
New families to the district are invited to attend from 3:00–4:00 PM, and all families are invited from 4:00–6:00 PM.
During Open House, you will have the opportunity to meet your teachers, find your classrooms, and move into your lockers.
As a reminder, the first day of school is September 1, 2026.

Important Clinic Information:
Dear Parents/Guardians,
As the 2026-2027 school year approaches, I would like to share important health information to help you and your child prepare for a safe and successful year. Please review the information below carefully.
🏥 Medical Information
Please notify both the nurse and your child’s teacher(s) of any medical conditions or concerns.
Be sure to update all medical information and emergency contact numbers in Final Forms so we can provide the best care in the event of an emergency.
Without parent/guardian consent on file, the school is unable to provide specific screenings or non-emergency health services. Families should seek follow-up through their healthcare provider as needed.
💊 Medications
If your child requires medication during the school day, including emergency medications such as an EpiPen, inhaler, or seizure medication, please follow the guidelines below.
Parents/Guardians are responsible for:
Submitting a completed medication authorization form signed by both a medical provider and parent/guardian before medication can be administered at school
Delivering medication to the school in its original container
Ensuring prescription medication is properly labeled by a pharmacist or healthcare provider
Providing medication that is current (within the past 12 months) and replacing it prior to expiration
Administering the first dose of any new medication at home (unless it is an emergency medication)
Notifying the school promptly of any changes to medication or healthcare providers
Providing an updated authorization form if there are any changes to the medication or dosage
The Authorization for Administration of Medication Form will be linked below and available at Open House on August 20th.
💉 Immunizations
All incoming 12th grade students are required by law to receive the following vaccines: 2nd dose of MCV4 (Meningococcal ACWY)
Immunization records or a signed exemption form must be submitted by September 15th.
Please contact your child’s healthcare provider or local health department if your child needs immunizations.
💉 Need Immunizations?
The Sandusky County Health Department will be available during Open House:

🥗 Dietary Needs
If your child has a food allergy, please contact Michelle Guzman and complete the required form.
A physician and parent/guardian signature are required.
The Dietary Needs Form will be attached below.
🚫 When to Keep Your Child Home
Please keep your child home from school if they have:
A fever within the past 24 hours (without the use of medication)
Vomiting or two or more episodes of diarrhea within the past 24 hours
A persistent cough, sore throat, or stomachache that interferes with learning
Pink eye
Head lice (must be treated before returning to school)
👂👁 Hearing & Vision Screening
Screenings are conducted for students in PK, Kindergarten, 1st, 3rd, 5th, 7th, 9th, and 11th grades.
If consent is declined in the Parent Bill of Rights on Final Forms, your child will not be screened.
🩺 Health Plans
If your child has an existing health plan:
You may stop by during Open House or the week before school to review and sign the plan
To schedule a meeting, please call between 9:00 AM–2:00 PM the week before school starts
Unless changes are needed, last year’s health plan will remain in place
🦷👓 Health Partners of Western Ohio
Lakota Local Schools has partnered with Health Partners of Western Ohio (HPWO) to provide dental and vision services during the school day.
Services include dental exams, cleanings, fluoride treatments, sealants, as well as vision exams and glasses if needed.
All students may participate with parent/guardian permission. These services are available to both insured and uninsured students with no out-of-pocket cost to families. Insurance will be billed if available but is not required.
To participate, a Health Partners consent form must be completed. A link will be provided below, and paper copies will be sent home with elementary and middle school students.

It's a great time to be a Raider! Fall Sports is set to Kick Off soon and we could not be more excited! Here are a few important upcoming dates to get on your calendar:
Saturday August 1 - first day or required practice in Football, Volleyball, and Cross Country. Golf starts July 29
Wednesday August 5 - Required Parent Meeting at 7:00 pm in the Auditeria
Monday August 10 - Sports Picture Day - from 2:30 to 6:30 pm (More details coming soon)
Wednesday August 12 - Required Drug Testing for Fall Sports Athletes grades 7-12 - 2 - 4 pm in the Auditeria
Lastly, we are expecting some of the best participation numbers we have seen in recent years. Kids are having fun and working hard. Coaches are working hard and excited! It's not too late for those of you students sitting on the fence. Come out for sport and enjoy the experience of being apart of something memorable!

***THE OPEN ENROLLMENT APPLICATION PROCESS HAS CHANGED***
Attention parents and students in the State of Ohio wishing to attend Lakota Local School District, Sandusky County, tuition free. Lakota will once again participate in OPEN ENROLLMENT.
We are no longer using a separate service for the application process. All Open Enrollment Applications and documentation for residency will be submitted online through Final Forms. Anyone wishing to participate in this program may apply online at https://lakotasandusky oh.finalforms.com/ or by clicking the FinalForms tab on the district homepage beginning April 1, 2026.
In addition to a completed online application, you must provide PROOF OF RESIDENCY. You will also find information on how to access Final Forms and upload documents for proof of residency on the Home Page of Lakota’s website in the District Tab under Enrollment/Registration.
Non-resident students can complete an application to attend the Lakota Local School District at no cost. We hope you plan to attend Lakota Local Schools again this school year. Applications are being accepted online by way of Final Forms through April 30th for the 2026- 2027 school year.
Please complete the application via FinalForms and upload a current copy of Proof of Residency.

If you missed the spring Kindergarten Registration, your child will be assessed during the first week of school. Please be sure to complete the required forms prior to August 20th in order to be assigned a classroom teacher.
